We are a local, independent, non-profit providing home health, physical therapy, palliative and hospice care to meet the diverse needs of 32 surrounding communities in central and northern New Hampshire.

Through the support of more than 70 employees and volunteers, Pemi-Baker touches lives in a variety of ways: helping you return home comfortably after surgery, strengthening your knee through physical therapy, or supporting you and your family with advanced illness at home.

Whether returning home from the hospital, a skilled nursing facility, or a doctor’s visit, we help our patients safely transfer home with confidence and achieve independence in their community. We help make hospital stays shorter and less frequent by providing healthcare in the home.
