SMS Privacy Policy and Terms & Conditions

This Privacy Policy explains how Pemi-Baker Hospice & Home Health collects, uses and discloses personal information about our customers, prospective customers, and visitors to our website at www.pemibakerhospicehomehealth.org. SMS opt-in or phone numbers for the purpose of SMS are not being shared with any third party and affiliate company for marketing purposes.

  • We use Ring Central for our text message communications from Pemi-Baker Hospice & Home Health employees regarding scheduling.
  • We use Google Analytics to help us understand how visitors interact with our website.

Mobile Messaging Terms & Conditions

Effective Date: 9/27/2024, last revision 1/17/2025

Pemi-Baker Hospice & Home Health operates a mobile messaging program (Program), subject to these Mobile Messaging Terms and Conditions. The Program and our collection and use of your personal information is also subject to our Privacy Policy. By enrolling, signing up, or otherwise agreeing to participate in the Program, you accept and agree to these Mobile Messaging Terms and our Privacy Policy. If you do not agree with these terms, please do not engage with our services. SMS opt-in or phone numbers for the purpose of SMS are not being shared with any third party and affiliate company for marketing purposes.

  1. Program Description: Pemi-Baker Hospice & Home Health may send promotional and transactional mobile messages in various formats through the Program. Promotional messages advertise and promote our products and services and may include upcoming events and other marketing offers. Transactional messages relate to an existing or ongoing transaction and may include appointment reminders from our agency staff or information about your employment application status, onboarding materials, or other employment-related updates. Mobile messages may be sent using an automated technology, including an autodialer, automated system, or automatic telephone dialing system. Message frequency will vary but will not exceed 10 messages per month. You agree that Pemi-Baker Hospice & Home Health, our affiliates, and any third-party service providers may send you messages regarding the foregoing topics or any topic and that such messages and/or calls may be made or placed using different telephone numbers or short codes, except in connection with marketing purposes. Pemi-Baker Hospice & Home Health does not charge for mobile messages sent through the Program but you are responsible for any message and data rates imposed by your mobile provider, as standard data and message rates may apply for short message service and multimedia message alerts.
  2. User Opt-In: By providing your mobile phone number to Pemi-Baker Hospice & Home Health, you are voluntarily opting in to the Program and you agree to receive recurring mobile messages from us at the mobile phone number associated with your opt-in, even if such number is registered on any state or federal “Do Not Call” list. You agree that any mobile phone number you provide to Pemi-Baker Hospice & Home Health is a valid mobile phone number of which you are the owner or authorized user. If you change your mobile phone number or are no longer the owner or authorized user of the mobile phone number, you agree to promptly notify Pemi-Baker Hospice & Home Health at info@pbhha.org or 603-536-2232. Your participation in the Program does not require that you make any purchase from Pemi-Baker Hospice & Home Health and your participation in the Program is completely voluntary.
  3. User Opt-Out and Support: You may opt-out of the Program at any time. If you wish to opt-out of the Program and stop receiving mobile messages from Pemi-Baker Hospice & Home Health, or you no longer agree to these Mobile Messaging Terms, reply STOP, QUIT, CANCEL, OPT-OUT, and/or UNSUBSCRIBE to the mobile number the message is sent from. You may continue to receive text messages for a short period while Pemi-Baker Hospice & Home Health processes your request and you may receive a one-time opt-out confirmation message stating that no further messages will be sent. You understand and agree that the foregoing is the only reasonable method of opting out. If you want to join the Program again, just sign up as you did the first time, and we will start sending messages to you again. For support, reply HELP to any mobile message from Pemi-Baker Hospice & Home Health or contact us at info@pbhha.org or 603-536-2232.

Pemi-Baker Hospice & Home Health’s mobile messaging platform may not recognize requests that modify the foregoing commands, and you agree that we and our service providers will not be liable for failing to honor requests that do not comply with the requirements in these Mobile Messaging Terms. Pemi-Baker Hospice & Home Health  may also change the telephone number or short code we use to operate the Program and we will notify you of any such change. You acknowledge that any requests sent to a telephone number or short code that has been changed may not be received by Pemi-Baker Hospice & Home Health and we will not be responsible for failing to honor a request sent to a telephone number or short code that has been changed.

  1. Disclaimer of Warranty and Liability: The Program is offered on an “as-is” basis and may not be available in all areas, at all times, or on all mobile providers. You agree that neither Pemi-Baker Hospice & Home Health nor our service providers will be liable for any failed, delayed, or misdirected delivery of any mobile message or information sent through the Program.
  2. Modifications: Pemi-Baker Hospice & Home Health may modify or cancel the Program or any of its features at any time, with or without notice. To the extent permitted by applicable law, we may also modify these Mobile Messaging Terms at any time. Any such modification will take effect when it is posted to our website with an updated revision date. You agree to review these Mobile Messaging Terms periodically to ensure that you are aware of any modifications. Your continued participation in the Program will constitute your acceptance of those modifications.

SMS TERMS AND CONDITIONS

  1. SMS For Consent Communication: The Phone Numbers obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.
  2. Types of SMS Communications: If you have consented to receive text messages from Pemi-Baker Hospice & Home Health, you may receive text messages related to: (Appointment reminders and follow-ups on cases.)  For Example: Hello Thank you for contacting Pemi-Baker Hospice & Home Health, how can we help you? Reply Stop to opt-out at any time. Message and data rates may apply, for assistance, text Help or visit our [Privacy Policy] and [Terms of Service].
  3. Message Frequency: Our SMS message frequency will be from 50 to 100 text messages daily across all users.
  4. Potential Fees for SMS Messaging: Many carriers charge a fee for each message sent or received. This can vary depending on the carrier’s pricing structure and whether the message is sent domestically or internationally.
  5. Opt-In Method: Customer will Opt-In for SMS messaging from Pemi-Baker Hospice & Home Health through texts or our website [Contact Us], this agreement for SMS will not be shared with third parties for marketing purposes.
  6. Opt-out: Customer will be able to opt out of SMS messaging from Pemi-Baker Hospice & Home Health by replying STOP at any time to any received SMS message, once opted-out they will receive no further SMS communication. They can Opt back In at any time by replying START.
  7. Help: If you are experiencing any issues, you can reply with the keyword HELP. Or, you can get help directly from us at info@pbhha.org
    Additional Options: If you do not wish to receive SMS messages, you can choose not to check the SMS consent box on our forms.

The Rewards of Being a Hospice Volunteer

While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health (PBHHH) since October 2021.  When I retired, I began searching for volunteer work that would allow me to give back to my community.  A newsletter from PBHHH arrived in my email announcing training for hospice volunteers.  My family had experienced the need for hospice twice, once in Ohio and again in Florida.  The relief my family and I felt when the hospice volunteers arrived was beyond measure.  I want to bring the same kind of assistance to others.  PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.  Their guidance has enabled me to meet the challenges of being s hospice volunteer, and I am very glad that I did it.
